We will assist in the completion and filing of your insurance claims, or if you wish, we will provide a statement containing all the necessary information for you to submit your claim. To process insurance claims, we need to obtain all current insurance information at the time of your appointment.
Patients with insurance requiring co-payment are requested to pay the co-payment at the time of the service. If you have any questions about your bill, or have a temporary financial problem, please discuss these with our patient accounts representative.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient. A $75.00 charge may be applied if you do not provide us with 24 hours notice.
