To ensure you receive the best care possible, your visit will be thorough and personalized. We kindly request you plan to be at your appointment for 2 hours.

We may run late because of emergencies, delays in surgery, or the discovery of a more severe problem that can take additional time.

When your turn comes, be assured that you will be given a thorough examination and time for explanation and questions.

Check-in Process

What type of exams do you do?
  • Medical Eye Exam
    • You may receive a thorough dilated exam to address medical eye conditions such as: Cataract, Glaucoma, Diabetic Retinopathy, Corneal Dystrophies, and Macular Degeneration.
    • This type of exam is a detailed medical exam to determine, assess, and recommend any treatment necessary and may include additional testing.
  • Specialty / Therapeutic Contact Lens Exam
    • You will receive a comprehensive eye examination to assess your suitability for specialty contact lenses. This exam is designed to address unique eye conditions or vision needs that cannot be corrected with standard contact lenses such as: keratoconus and corneal irregularities.
    • This exam typically includes detailed measurements of the cornea, tear film analysis, and careful selection of the appropriate lens type to best fit your specific needs. This exam requires a more precise fitting process to ensure comfort and optimal vision correction.
How long do should I expect to be at my appointment?

To provide the best possible care, your visit will be very thorough. We kindly request you plan to be at your appointment for 1 to 2 hours.

How early do I need to arrive before the appointment?
  • If you have completed your paperwork prior to your appointment, we recommend arriving 5-10 minutes early.
  • If you have not completed your paperwork prior to your appointment, we recommend arriving 15-20 minutes early to complete your forms.

Will my eyes be dilated?

For most appointments, your eyes will be dilated. There are a few appointments that do not require dilation. When your eyes are dilated during your visit, it will make your vision blurry and your eyes more sensitive to light for several hours. If you are not using public transportation, please consider arranging for someone to drive you home. Call our office if you have questions or are unsure if your appointment requires dilation.

What should I bring to my appointment?
  • Insurance card and photo ID (for billing and identification purposes)
  • Co-Payment, if applicable
  • List of medications
  • Eyeglasses, and/or eye drops
  • Contact lens prescription or contact lens box
  • Your completed registration forms (You can access the forms here)
Will I have surgery?

Please understand that your initial appointment is an evaluation and consultation. You will NOT have surgery during this appointment. If a procedure is necessary, one will be scheduled during this consultative appointment.

Can I bring a family member or friend?

If you plan to discuss your treatment with a relative or friend, we encourage you to have him or her with you during your exam and to be placed on your HIPAA form when you check in for your appointment. You can access the HIPAA form here.

What insurance do you take?

Levenson Eye Associates participates in most health care plans. Plans change frequently, so please check with your insurance provider to verify we accept your plan. If your insurance plan is one we accept, please bring your insurance card and any applicable co-payments with you to your appointment. If a referral authorization is required, be sure to contact your insurance provider beforehand. If you have any questions, feel free to give us a call—we’re happy to help!

What is your Billing & Fees process?
  • Payment is requested at the time the service is provided. Patients with insurance requiring co-payment are requested to pay the co-payment at the time of the service. If you have any questions about your bill, or have a temporary financial problem, please discuss these with our patient accounts representative.
  • We accept the following credit cards: Visa, MasterCard, Discover and American Express.
Is there a cancellation fee?

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
